Denver Regional Council of Governments invites partners to join Phase 2 of Health at Home Challenge

DRCOG wins first phase of competition through its innovative approach to delivering community-based services

The Denver Regional Council of Governments is inviting community partners and healthcare organizations to join the second phase of the Administration for Community Living's Health at Home Challenge. This multi-phase initiative will support advanced community care networks as they provide affordable and comprehensive health services for older adults and people with disabilities so they can remain in their homes and communities. DRCOG's Area Agency on Aging is the designated Community Care Hub for Colorado.

The first phase of the Health at Home Challenge focused on strategies that reach many higher-need and eligible Medicare and Medicaid beneficiaries through partnerships. DRCOG was one of the winners of Phase 1, which ran from April to July 2026, because of its innovative approach to delivering community-based services. DRCOG was awarded $200,000 and will use those funds to pay for community-based services for the people referred to the Community Care Hub by healthcare partners.

“We are honored that the DRCOG Community Care Hub has been selected as a Phase 1 winner in the Health at Home Challenge,” said AJ Diamontopoulos, manager of integrated health at the Area Agency on Aging. “Over the past several years, our team has worked to strengthen our technology, data-sharing capabilities, and staff expertise so we can better connect people with the services they need. We are grateful to the Administration for Community Living for this recognition and to our healthcare partners, including UCHealth and CommonSpirit Health, for their commitment to addressing their patients’ social needs. We hope this opportunity will also encourage other healthcare providers to partner with us as we continue building a stronger, more connected system of care.”

The second phase of the Health at Home Challenge is now running through July 2027 and will focus on the implementation of those strategies, with demonstrations of how the advanced community care networks coordinate and manage care, while improving reach and volume. This includes using shared infrastructure, such as technology, administration and financial services. A total of up to $2 million in prizes will be awarded to five winning teams that best meet the judging criteria.
