
Through the Options Counseling and Case Management programs, Area Agency on Aging case managers provide intensive, short-term, services for eligible people 60 and older, or 18 and older with a disability and their caregivers. They work with individuals to determine their wishes and needs, create a plan for the future, then help them access resources, information and services to stay as safe and independent as possible.
Case management activities can include:
- Conducting a comprehensive needs assessment to develop a client-centered care plan.
- Presenting care options and resources.
- Coordinating services among providers.
- Connecting with medical, nonmedical and mental health professionals and services.
- Applying for and accessing benefits and resources.
- Consulting with families and temporarily monitoring community resources.
Some case managers speak Spanish, and case managers use interpretation services for additional languages.
