Medicaid Transition Services Options Counseling

Medicaid Transition Services Options Counseling is a program of the Area Agency on Aging providing options counseling for Medicaid members living in skilled nursing facilities, Regional Centers and intermediate care facilities.

Options counseling helps long-term residents of local skilled nursing facilities and other qualified facilities learn the options available for community living. Options counselors will provide information necessary to determine whether moving back into the community is right for each client.

During an options counseling meeting, participants will receive:

  • A personalized needs assessment.
  • Information about community services including Medicaid transition services like targeted case management-transition coordination, Home and Community-Based Services and the Program for All-Inclusive Care for the Elderly.
  • Referrals to community services.

Make a referral

  • Contact 303-480-6838.
  • Fax a referral form to 303-480-6827. (Nursing home residents should contact their social worker for a referral form or ask the social work to complete a form on the client's behalf.)
  • Complete the Medicaid Options Counseling Referral form.
