Medicaid Transition Services Options Counseling is a program of the Area Agency on Aging providing options counseling for Medicaid members living in skilled nursing facilities, Regional Centers and intermediate care facilities.
Options counseling helps long-term residents of local skilled nursing facilities and other qualified facilities learn about options available for community living. Options counselors will provide information to clients and their caregivers to understand how Medicaid Transition Services can support their goals to return to community living.
During an options counseling meeting, participants will receive:
- A personalized needs assessment.
- Information about community services including Medicaid transition services such as Transition Coordination, Money Follows the Person, Home and Community-Based Services and the Program for All-Inclusive Care for the Elderly, also known as PACE.
- Referrals to community services.
