Colorado Choice Transitions

Colorado Choice Transitions (CCT) is a program of the Area Agency on Aging that provides two types of services, Options Counseling and Community Transition Services/Extended Transition Services
Options Counseling, a program designed to help long-term residents of local skilled nursing facilities learn what options might be available for community living. Our Options Counselors will provide you with the information and decision support necessary to see if moving back into the community is right for you.
At your ADRC Options Counseling meeting, you’ll receive:
  • a personalized needs assessment
  • information about community services including Medicaid transition services like Colorado Choice Transitions, Home & Community-Based Services (HCBS), and the Program for All-Inclusive Care for the Elderly (PACE)
  • referrals to community services
Anyone can make a referral to Options Counseling. To make a referral, please call 303-480-6838 or fax a referral form to 303-480-6827 (you can ask your social worker for a referral form or they can complete one for you).
Community Transition Services/Extended Transition Services, which helps individuals living in skilled nursing facilities determine if transition to a community-based setting works for them. Our staff can:
  • Coordinate your transition into a community-based setting
  • Help access state plan benefits, home- and community-based waiver services (HCBS) and, if applicable, Colorado Choice Transition (CCT) enhanced services
  • Improve access to an array of HCBS services
  • Increase housing options for people with all types of disabilities
  • Support individuals transitioning through the CCT program for the first five months after the transition occurs (up to the first full year if needed)
  • Navigate benefits, housing, and community service systems and organizations
  • Provide access to supports, services, tools, and information needed to successfully and safely remain in the community
